Billing Options

  • Our office is in network with CHP, Medicare Part B and United Health Care (Commercial Plans Only). However, we will gladly accept patients with out of network insurances and require the Patient Acknowledgement From - Price Guide be completed and returned to the office prior to your appointment. We are not able to file insurance claims for out of network insurances, if the patient would like to file an insurance claim themself our office can provide the necessary information.
  • As a courtesy, we will file claims to in network insurances only for the patient. The office will not obtain prior authorizations for office visits.
  • Co-pays are due on the date of service. The complete fee for service is due on the date of service for patients with out of network insurances. Please note that payment on patient accounts is patient responsibility regardless of insurance coverage.
  • We accept cash, checks, and credit/debit cards with the Visa, MasterCard, Discover, or American Express logo.
