New Patient Information
How to Become a New Patient
We are currently accepting new patients into our practice. All new patients are required to complete a new patient intake form, which will be reviewed by the corresponding provider and a determination will be made as to whether or not an appointment at our office will be beneficial to you. If you wish to proceed with the process of becoming a new patient we ask that you either call our office (850-205-0189) to complete the intake on the phone, or click here to print and complete it at home. Please fax, mail or drop off the completed form to our office, should you choose to fill it out at home.
For Scheduled New Patients
If you have already scheduled a new patient appointment, please note that we ask for you to arrive thirty minutes prior to your appointment time to complete new patient paperwork. If you do not arrive early enough and don't have time to complete the paperwork your appointment is subject to being rescheduled as it will shorten your scheduled time with the provider. You may complete the paperwork at home and bring it with you to your appointment, in which case you may arrive five minutes before your scheduled appointment time to allow time to process your insurance information. Please print the following documents, answer all questions completely and bring with you to your scheduled appointment, if you wish to complete the paperwork in advanced.

Please note that the last page of this document is a copy of our office policies, for you to review and keep. Please also note that we will contact you to confirm your new patient appointment two business days in advanced and if we do not get a confirmation by noon the day before, your scheduled appointment will be canceled. We require a 24-hour notice in cases of all cancelations.
